Our Multicultural Aged Care Navigators can help you, in your preferred language, to access accurate and independent information on how to access aged care supports. Aged care support can include social activities, help with shopping, help with showering, meal delivery, nursing care and more. Aged care supports can include care given to you at home or can be in a residential facility.
We can provide culturally-appropriate and tailored information about your aged care support options and one-on-one support to help you access My Aged Care, which is the Australian Government’s website for accessing aged care supports. If you are already registered with My Aged Care, we can also help you change the type of aged care service you receive.
If you need another kind of help, we can also connect you to someone else who may be able to help you.
Our Aged Care navigators can speak Cantonese, Mandarin, Filipino, and English. If you speak another language, let us know and we can arrange someone to assist you.
The services are free and available to people aged 65 and older from non-English speaking backgrounds, their carers, and families.
You can refer yourself, a family member or friend simply by printing and completing this form and returning it to us in person or by email on agedcarenavigation@worldwellnessgroup.org.au or complete the online referral form.
We are closed on Saturdays, Sundays and public holidays. When we are closed, we recommend the National Home Doctor Service on 13 74 25 or if it’s an emergency, call 000.
