The MHS Referral Form 2020 can be downloaded here, or alternatively, we can email one to you upon request.

Anyone can contact us to make a referral. You can contact us by filling the form or call us on 07 3333 2100 to seek support for yourself or if you are concerned about a family member or friend. You may email mentalhealth@worldwellnessgroup.org.au
