Culture in Mind – Wraparound Model of Care
Our approach to delivering service is thoroughly client-centred and informed by our extensive experience working with CALD communities. This ensures people’s needs are met in a culturally-appropriate and collaborative manner, acknowledging the connections people have to their families and wider community. We call this approach the Wraparound Model of Care, as illustrated in the following graphic:
Individualised
Our Wraparound support is individualised to meet the needs of the person, including their cultural needs.
Strength Based
Our Wraparound support actions are focused the person’s strengths, and goals and aspirations, while building resilience and meeting practical needs. The Wraparound processes and the plan identify, build on, and enhance the capabilities, knowledge, skills, and assets of the person.
Culture Based
Our Wraparound processes and actions demonstrate respect for and builds on the values, preferences, beliefs, culture, and identity of the person, their family and their carer or community.
Family and Carer Voice
Our Wraparound processes recognise the importance of valued connections between people, family and carers. Hence, if the person agrees, planning is grounded in family and carers’ perspectives, values and preferences.
